FACT:
Pilots make errors.
Years ago, the aviation community tried to reduce accidents by eliminating errors, an impossible task.
Threat and Error Management (TEM) shifts the focus to anticipating/managing threats and monitoring for and trapping errors before they result in an accident.
The C-A-B’s of Threat and Error Management
Communication
It is important for crews to communicate effectively. Communication puts crews on the same page, raises situational awareness, and maximizes barriers and SOPs.
Actively Assess/Monitor
Anticipate threats before they happen. Communicate a plan. Actively assess and monitor aircraft flight path. Pilots will make errors, but TEM will minimize the chance they occur.
Barriers
Put barriers in place to TRAP errors. Barriers may include SOPs, checklists, external resources or a crews knowledge/experience.
